Sydney Water Health and Wellbeing Self-Assessment

This assessment will ask you to provide information about your current health including information regarding existing health/medical conditions to aide in determining recommendations to assist you to manage risk and continue to work safely.

At the completion of the self-assessment you will be provided with a recommendation based on the information you have provided, these recommendations can include: No action required, referral to nurse triage, referral to telehealth (Doctor or psychologist). If further services are recommended Injurynet will contact you to facilitate a nurse triage or telehealth appointment at your convenience.
